The Latinos and Italians are in full control of the drug trade as Burr Redding is too distraught over the death of Augustus Hill to lead the Homeboys. Guerra finds himself in a knife fight with the former leader of El Norte, Jose Torres, who is back in Oz presumably for parole violation. Guerra is nearly stabbed in the neck before COs break the fight up. When Guerra is asked by McManus why Torres was trying to stab him, Guerra tells McManus that he does not know and that Torres was just crazy. His near death experience prompts Guerra to reflect on his life in Oz and the aggressive way he carries himself. Miguel Alvarez is soon after brought back into Emerald City where Guerra is given another warning by Tim McManus. McManus points out that every discipline problem Miguel has had has somehow been tied to Guerra and that he will hold him responsible for any bad thing that may happen to Miguel. Surprisingly, Guerra decides he wants to be friends with Miguel again and to end their long running feud, and the two of them make peace gradually.

Morales is soon taken down by CO Dave Brass whom he caused an injury to a year prior, and dies in the hospital alongside Carlos Martinez. Pancamo then asks Guerra who will lead El Norte and Guerra says Alvarez is the best choice. Miguel refuses though as he wants to be paroled. Shortly afterwards, a gay nightclub owner Alonzo Torquemada comes to Oz with a pill form of heroin and ecstasy called D-Tabs, also referred to as Destiny. These drugs are popular at gay nightclubs. Guerra takes some of these drugs while listening to Techno music and begins ripping off his own flesh under the influence and is sent to the infirmary. You can see Guerra rolling out of the infirmary in a wheelchair in the last episode. It can be assumed that he was evacuated from Oz after the Anthrax situation. His survival establishes himself as one of the last original members of El Norte to survive the entire series run, and one of the few secondary characters to survive.
