1939 Atlantic Ocean Imperial Airways Short Empire Flying Boat Sinking - Report

Report

The British Air Ministry's Inspector of Accidents reported that the accident had been caused by icing in the carburetors of all four engines. This caused a full loss of power in the inboard engines and partial loss in the outer; the commander of the Cavalier had reported icing problems prior to ditching. The inspector recommended that extra heating of carburetors and of the incoming air be provided and that a temperature indicator be installed. He also advised that passengers should be instructed in the fastening of lifebelts and the location of emergency exits and recommended the provision of extra life-saving equipment like rafts and pyrotechnic signals and that passengers should fasten safety belts at take-off and alighting.

Famous quotes containing the word report:

What a wise and good parent will desire for his own children a nation must desire for all children.
—Consultative Committee On The Prima. Report of the Consultative Committee on the Primary School (HADOW)

The report reflects incredibly terrible judgments, shockingly sparse concern for human life, instances of officials lacking the courage to exercise the responsibilities of their high office and some very bewildering thought processes.
Jane Jarrell Smith, U.S. widow of American astronaut Michael J. Smith. As quoted in Newsweek magazine, p. 13 (June 30, 1986)

How easily some light report is set about, but how difficult to bear.
    Hesiod (c. 8th century B.C.)
