Despite its age and high usage, train related accidents which have involved passenger fatalities are exceptionally rare, the last major incident to result in these being the Moorgate tube crash in 1975. Derailments in the period 2003–2004 did not result in fatalities, although the Chancery Lane derailment in 2003 led to a closure of the Central Line whilst urgent safety checks were undertaken.

A number of major incidents have taken place in London Underground stations however:

  • 7 July 2005 London bombings: 39 civilians and 3 terrorists died.
  • 18 November 1987: the Kings Cross fire of 1987 (31 killed)
  • 3 March 1943: the Bethnal Green crush disaster (173 killed; the as yet unopened station was being used as an air raid shelter)
  • 11 January 1941: Bank station (London) was hit by a bomb (56 killed)
  • 12 November 1940: Bomb falls on Sloane Square tube station (36 killed)
  • 13 October 1940: A German bomb falls on Bounds Green tube station killed 16 people.
  • 14 October 1940: Balham tube station was flooded as a consequence of bomb damage, killing 68,
  • 17 September 1940: Marble Arch tube station (20 killed by German bomb)
