Karam Apnaa Apnaa is the story of Gauri and of the twists and turns of her destiny. Gauri is a charming and innocent middle-class Bengali girl of marriageable age. She has lost her mother long ago. Her father and a wealthy Punjabi businessman, Mahen Kapoor, are best friends in spite of the huge difference in their cultures and status. Mahen's parents and his wife Nikhila dot on Gauri. With Mahen's help, Gauri receives a proposal of marriage from Shashank. Gauri and Shashank get engaged and are about to get married. On the day of the marriage, Gauri is shocked to see Mahen's arrogant son Shiv Kapoor as her bridegroom. She is shattered on learning that Shashank has refused to marry her and the Kapoors have forced Shiv to stand in as the bridegroom. Unable to accept the situation, she runs away from the marriage venue. That's when she meets Anupam, who advises her to face the problem instead of running away from it. She returns to the wedding venue, much to the anger of Shiv, who was hoping that she would back out. Shiv and Gauri get married, and he greets her by saying, "welcome to hell!". However, he cannot resist the simplicity and purity of his wife. Before long, his hatred gets converted into friendship and regard.

After the marriage, a series of mishaps happen to Gauri. Everyone assumes it to be an accident; however the truth is something else. It is revealed that Shashank's disappearance and Shiv-Gauri's wedding were planned by Mahen's wife Nikhila. This was because Shiv's kundali had a major flaw dosh, which foretold that his first wife would die soon after their wedding. When the truth comes out, everyone advises Gauri to leave Shiv, but she refuses as she has fallen in love with him. Gauri's love becomes a source of guilt for Shiv, because he is cheating her for his childhood sweetheart Ipshita. It was to protect Ipshita from the ill-effects of his stars that Shiv had agreed to marry Gauri. However, fate has different plans in store for him and brings Gauri close to him. The family preceptor, Guruji, advises Nikhila that Shiv's wife will die on the 31st of January. The fateful date is approaching. Feeling confident that Gauri's days are numbered, Nikhila convinces Ipshita to get engaged with Anupam(who is her admirer), so that no one will suspect the truth. Gauri meets with a major accident but survives. Now Ipshita turns insecure and pressurizes Shiv to divorce Gauri. However, Shiv is unable to do so, because he learns that Gauri is carrying his child. Ipshita gets married to Anupam when she realized that Shiv will not leave Gauri.

The story takes a 5 years leap. Shiv-Gauri are the proud parents of twin sons Om & Priom as well as an adorable daughter Shanti. Shiv believes he is happy, but still remembers his first love. Ipshita is now a writer and has left Anupam. Gauri is a doting mother who cannot spare enough time for her husband. Nikhila wants Ipshita back in Shiv's life, the reason being something to do with property. Mahen and Nikhila's son, Samar, who had been kidnapped in childhood, is now found. He is Shiv's identical twin. Nikhila calls Samar to the Kapoor mansion where he is mistaken for Shiv. He misbehaves with Gauri and the kids and is successful in getting the documents of the property signed by Gauri. Nikhila and Ipshita join hands with Mahen's sister Maya. Gauri is shocked to know that Nikhila is planning to separate her from Shiv; however, she is unaware of Ipshita's intentions. Ipshita takes advantage of Shiv's vulnerability and has a one-night-stand with him. Gauri is successful in exposing Nikhila's evil motives. Nikhila is thrown out of the Kapoor Mansion. Nikhila, Ipshita and Maya, with the help of doctors, fake a report that Gauri is suffering from a brain tumor.

Meanwhile Gauri gets to know the truth of Shiv's affair and learns that Ipshita is pregnant. Expecting an early death for herself, Gauri forces Shiv to get married to Ipshita. However, on the day of the wedding, Gauri realizes that she has been tricked. She swears to throw Ipshita out of Shiv's life. Samar helps her in this effort. Shiv eventually gets to see Ipshita's true colours and throws her out. Meanwhile, Nikhila has a change of heart and re-enters the Kapoor family. Her return is followed by the reappearance of Shashank, along with Mahen's nephews Moksh and Asim with their evil maternal uncle Mamaji. Samar is madly in love with Gauri's younger sister Kaya. The two lovers plan to get married. However, Shashank cunningly fools Samar and himself gets married to Kaya. Shashank's real motive is to take revenge from Shiv and acquire the property of the Kapoors.

One night as Gauri and the children are waiting for him to come home, Shiv is brutually murdered by Moksh, Asim and Shashank. Gauri is initially kept in dark about his death, but eventually learns the truth. Shashank, Moksh, Asim and Mamaji continued to harass and trouble the Kapoors. Then, Samar came face-to-face with his dead twin Shiv's ghost. Samar comes to know how Shiv died and vows to take revenge. Samar and Gauri bring Shiv's killers to book in the court of law, but Shashank escapes. Shiv's ghost also disappears after this. Now, family members start pressuring Samar to get married to the widowed Gauri in order to provide support to her and Shiv's children. However, Samar and Gauri reject the idea. Shashank starts blackmailing Gauri for the property and she is about to reveal the truth to Nikhila; however, in an unexpected turn of events, Nikhila is murdered. Samar thinks Gauri has murdered Nikhila and Gauri suspects him. Gauri runs away and is assumed to be dead.

A few years later, Samar has turned into a cold-hearted business tycoon, and Gauri, who has lost her memory, is living in another town under the name of Shivangi. Samar hates the memory of Gauri, whom he believes to be his mother's killer. Nisha, Anupam's sister, enters the scene to take revenge against the Kapoor family. Anupam is now paralyzed as Ipshita had tried to kill him in order to be with Shiv; Nisha blames the Kapoors for her brother's plight. Nisha makes a false accusation that Samar has taken sexual advantage of her. The Kapoor family pressurizes Samar to marry Nisha and he agrees. Meanwhile, Gauri regains her memory and decides to punish Samar for what she wrongly believes to be his crimes. She is aware of Nisha's true intentions. On the day of the marriage Gauri gets Nisha kidnapped and takes her place beside Samar as the heavily-veiled bride. The marriage rituals are completed. Samar is furious when he learns that he has been tricked into marrying Gauri, the woman he hates with a vengeance. They have many fights after that. Eventually, it is revealed that neither Samar nor Gauri killed Nikhila; the real culprit was Mahen's sister Maya, who wanted the Kapoor property. Now Samar and Gauri work out their differences and, slowly, their love blossoms. Then Samar feels unwell. Gauri's old friend Prateek is a doctor, and he diagnoses that Samar is suffering from a tumour. Samar doesn't want to see Gauri as a widow; so he asks Prateek to marry her. But, later, Samar is hospitalized and his tumour is cured.

Next, Samar's cousins-Vivaan and Dhruv usurp the entire family wealth. They throw Samar and Gauri out of Kapoor house. Because of this, Gauri and Samar migrate to Kolkata. They are poor but happy. That is, until the entry of Kamya, who falls in love with Samar. A lot of conflict ensues. Kamya and her mother poison Samar's mind and make him believe that Gauri is having an affair with Prateek. Gauri is carrying Samar's child; but he thinks it is Prateek's baby. Gauri decides to separate from Samar when she realizes that he doesn't trust her anymore. But Kamya's truth is eventually exposed. Meanwhile, Gauri delivers a baby girl. All the misunderstandings get cleared and Samar-Gauri are back in Mumbai. Prateek gets married to Aarti. A girl named Dhaani appears and claims Samar as her father. Later, it is revealed that she is the daughter of Kaya, who has been ill-treated by Shashank. Samar and Gauri move to London in order to expand their business. Dhruv and Vivaan continue to fight for the property. Eventually, Samar and Gauri come back to India and together solve the issue. The Kapoor family is re-united.
