The song debuted on the Billboard Hot 100 at number 67 and reached a peak position of three on the issue dated February 14, 1987, for a total of seventeen weeks spent in the chart. It had the same success in another charts, reached at number 4 in Hot Dance/Club Play Songs on the issue dated February 14, 1987. The song placed at sixty-one on year-end chart of 1987. In Canada, the song debuted at eighty-eight on the RPM issue dated December 13, 1986. After ten weeks, it reached a peak position of thirteen the chart, following two weeks, and spent a total of seventeen weeks in the chart. In the United Kingdom, "Change of Heart" debuted at number ninety-six on the UK Singles Chart, soon climbing to number sixty-seven Across Europe, "Change of Heart" peaked at number eight in France. Elsewhere, the song peaked at number forty-one in New Zealand.
