The nerve to piriformis originates in the sacral plexus. It arises from the ventral division of the second sacral nerve, or the ventral divisions of the first and second sacral nerves (S1 & S2), and enters the anterior surface of the piriformis muscle; this nerve may be double. Not to get confused by the inferior gluteal nerve which is also (S1 and S2).
