When it made its debut in 1937, State Route 665 was routed along its present alignment between the junction of U.S. Route 42, State Route 38, State Route 56 and State Route 142 in London and U.S. Route 23 near Lockbourne. Just two years later, the highway was extended on the east end, via a short jog northerly along U.S. Route 23, then east along what is today State Route 317 from U.S. Route 23 into Groveport, where the route came to an end at the Main Street intersection. Then, in 1973, a southward extension of State Route 317 through the eastern portion of suburban Columbus resulted in that highway being extended to Groveport. State Route 317 was linked in with what was then the eastern terminus of State Route 665, and then replaced State Route 665 from that point westerly to U.S. Route 23. Consequently, State Route 665 was restored to its original routing, and has not changed since that time.
