Shouha tababa, W-brona, W-ruha dqudsha,
min’alam w’adamma L-’alam, Amen.
Glory be to the Father and to the Son and to the Holy Spirit,
from everlasting and for ever and ever (one translation)

This form, as the cited sources show, is used by the Assyrian Church of the East and the Chaldean Catholic Church.
