Fishing Creek starts in State Game Lands Number 13, in the southern reaches of Sullivan County. The creek heads east until reaching Jamison City, where it turns south and heads towards Benton. From there, it follows Pennsylvania Route 487 south, passing through Stillwater, Orangeville, and Lightstreet. Near Stillwater, the creek picks up Huntingdon Creek and at Orangeville, picks up Green Creek. At Lightstreet, Fishing Creek turns southwest away from Pennsylvania Route 487, heading through residential/wooded areas next to Interstate 80. Fishing Creek then turns south near Buckhorn, picking up its final major tributary, Little Fishing Creek. The creek then heads south along the western border of the town of Bloomsburg until reaching the northwestern border of Bloomsburg, where it turns west and heads between Bloomsburg and Fernville. At the outskirts of Bloomsburg, Fishing Creek turns south and parallels Pennsylvania Route 42 to its mouth, the Susquehanna River.
