Six singles were released from the album. "Erotica" was released as the lead single of the album in October 1992. It peaked at number 3 on the Billboard Hot 100. The video, directed by fashion photographer Fabien Baron, was inspired by Andy Warhol. Following the release of the song, Leabanese singer Fairuz claimed her voice apppeared on the song without permission, also saying that the lyric "he crucified me today", which was sung in Arabic, are taken from a religious song that is traditionally heard during Eastern services. In its music video, Madonna portrayed a masked dominatrix with a golden tooth and a whip. Montages of sexual imagery are shown that are shot during the photography of Madonna's Sex book. It is a montage of sexually charged images, designed to shock the audience. MTV put the video into heavy rotaion, but only after midnight. "Deeper and Deeper" was released as the second single in November 1992 in the United Kingdom and in December 1992 in the United States. It received generally positive reviews from music critics. "Deeper and Deeper" achieved success in the United States, peaking at number seven on the Billboard Hot 100 and spent a total of seventeen weeks on the chart. "Bad Girl" was released in February 1993. The song was a modest success on the charts, reaching number thirty-six on the Hot 100.

"Fever" was released as the fourth single of the album in March 1993 in Europe and Australia. The song entered at its peak position of number six in the United Kingdom. In the United States, it became her fifteenth song to hit number one on the Billboard Hot Dance Music/Club Play chart. The music video for "Fever" portrays Madonna as a Balinese idol with red hair and another shot of her being silver painted. Interspersed with scenes of a man in thongs, his body painted gold and dancing in front of funky, kaleidoscopic backgrounds. The fifth single, "Rain" is the fifth single and was released in July 1993. "Rain" received rave reviews from music critics, who noted it as one of Madonna's best works, and was then compared to her aftermath single "I'll Remember", which was released a year later. "Rain" peaked at number fourteen on the US Hot 100 and number seven on the US Adult Contemporary charts. In the United Kingdom, the song was a success, peaking at number seven on the charts, selling 130,771 copies. Its accompaining music video portrays Madonna with short cropped, black hair, recording the song in a studio. Interspersed with scenes of Madonna kissing a man behind a glass on which water falls. Critical appreciation came for the music video, whose technical brilliance was awarded at the 1993 MTV Video Music Awards. "Bye Bye Baby" was released as the last single from the album in November 1993. The song received positive critics, and reached the top ten in Italy, and the top twenty in Australia and Japan. The music video for "Bye Bye Baby" shows live performances from The Girlie Show World Tour and from the 1993 MTV Video Music Awards.
