Route 91 takes the following route through the State:
- Westerly: 5.4 miles (8.7 km); Route 3 to Hopkinton town line
- Oak Street, Westerly-Bradford Road
- Hopkinton: 2.7 miles (4.3 km); Westerly town line to Richmond town line
- Alton-Bradford Road
- Richmond: 2.9 miles (4.7 km); Hopkinton town line to Charlestown town line
- Church Street
- Charlestown: 1.0 mile (1.6 km); Richmond town line to Route 112
- Alton-Carolina Road
