North Middlesex Regional High School has a long list of extra-curricular opportunities for students to take. The number of clubs, sports, and art programs available ensure that every student can find a way to get involved in the school outside of the classroom.

North Middlesex is known throughout the state for its award-winning music department. There are three vocal performing ensembles at North Middlesex. The women's choir, "Bella Voce," has received several gold medal ratings in recent years at state competitions. There are six instrumental performing ensembles, including the wind symphony, concert band, jazz band, jazz ensemble, marching band, and winter percussion ensemble. All of these ensembles have been highly rewarded in recent years for achieving a level of excellence unmatched by almost any other school in the state. The North Middlesex Tri-M Music Honor Society students also run Stars@Symphony, a two-day event that takes place at Mechanics Hall (Worcester, MA) and Symphony Hall (Boston, MA) which showcases bands that receive gold medal ratings from the Massachusetts Instrumental and Choral Conductors Association.

North Middlesex is also known for having an incredible Relay For Life. In recent years, their Relay has been recognized as one of the top fundraising Youth Relays in the United States.

The service learning opportunities at North Middlesex are unmatched by any other school in New England. Several service learning trips occur each year in which many North Middlesex students are given the opportunity to serve various communities. Trip locations have included New Orleans, New York City, and Philadelphia. There is also a community garden at the school which harvests impressive amounts of food for local food banks.
