Pope John Paul II created him Cardinal-Deacon of Saint Agnes in Agone on the 21 February 1998. He retired as president of he Administration of the Patrimony of the Apostolic See in November 1998 becoming President Emeritus of Administration of the Patrimony of the Apostolic See.
On 1 March 2008 Antonetti was elevated to Cardinal-Priest. His titular deaconry Sant'Agnese in Agone was elevated pro hac vice to title.
