Filling Out The Card
The vaccine certificate has columns to fill out with dates, nature of vaccine, dose, and physician's signatures. The date must be in the order of DD/MM/YYYY. It is common to see the physician complete the certificate using product name alone, and not indicate the "nature" of the vaccine.
