Investigation
Three investigations were launched after the incident: internally within QR; by Queensland Transport as rail safety in Queensland is regulated by the state government department, along with the Department of Industrial Relations' Division of Workplace Health and Safety and Electrical Safety Office; and the Australian Transport Safety Bureau to look at causes for the incident and to make recommendations to prevent it happening again.
